HIPPA (the Health Insurance Portability and Accountability Act) has changed the way we, Care Management Solutions, LLC, share information. We no longer share medical history, medication insurance information or current health conditions without a signed authorization from you the client, or verified Power of Attorney. This form must be completed each year in accordance with HIPPA regulations.
If you decide you want to share information with someone, simply complete the Authorized Representative Form, which we will happy to give you. The authorization is in effect for one year, unless we receive a written request from you to terminate the authorization.
